PRIVACY OF DATA SUPPLIED BY YOU

Vantage Pharmacy (Dispense4me Ltd) is committed to protecting your
privacy. We will only use the information that we collect about you
lawfully (in accordance with the Data Protection Act 1998). We collect
information about you for 2 reasons: firstly, to process your order and
secondly, to provide you with the best possible service. We will not
e-mail you in the future unless you have given us your consent. We will
give you the chance to refuse any marketing email from us or from
another trader in the future. The type of information vantage pharmacy
collects about you includes:

· Delivery address
· Telephone number
· Email address
We will never collect sensitive information about you without your
explicit consent. The information we hold will be accurate and up to
date. You can check the information that we hold about you by emailing
us. If you find any inaccuracies we will delete or correct it promptly.
The personal information that we hold will be held securely in
accordance with our internal security policy and the law. We may use
technology to track the patterns of behaviour of visitors to our site.
This can include using a "cookie" which would be stored on your
browser. You can usually modify your browser to prevent this happening.
The information collected in this way can be used to identify you
unless you modify your browser settings. We will not pass on any of
your details to third parties unless required to do so as part of the
order verification process or if required to do so by UK governing law.

paramount and Vantage pharmacy uses your information only as set out in

used for marketing purposes without first obtaining your consent.
CONSENT TO USE OF YOUR PERSONAL INFORMATION
By submitting your information, you consent to the use of that
information as set out in these terms and conditions of use. If we
change our terms and conditions of use, we will post the changes on
this page so that you may be aware of them and you will be given a
chance to ask us not to use your information for the changed purposes.
Because the Internet infrastructure is global, and it is not possible
to predict the routes that information sent over the Internet will
take, the information you provide may be transferred temporarily via a
route which takes it outside the European Economic Area as it passes
between you and us. By submitting your information you consent to this
transfer.
Freedom Of Information & Data Protection Acts
Pharmacists providing NHS services are required by 31 October 2003 to
submit a scheme for publication of certain information under the
Freedom of Information Act 2000.
This publication scheme is a complete guide to the information
routinely made available to the public by vantage pharmacy. It is a
description of the information about our pharmacists and pharmacy and
will be reviewed at regular intervals and we will also periodically
monitor its effectiveness.
How much does it cost?
The publications are all available to you free of charge unless
otherwise indicated within each class. Where information is provided at
a cost, the charges will be calculated as set out in Class 7 (This
Publication Scheme) overleaf and will be set out in £
sterling.

The information within each class is available in paper (hard copy)

YOUR RIGHTS TO INFORMATION
- In addition to accessing the information identified in the
Publication Scheme, you are entitled to request information about
vantage pharmacy under the NHS Openness Code 1995.
- The Freedom of Information Act 2000 recognises that members of the
public have the right to know how public services are organised and
run, how much they cost and how the decisions are made.
- From January 1st 2005 it will oblige the Pharmacy to respond to
requests about information that it holds about NHS Pharmaceutical
Services, and is recorded in any format and it will create a right of
access to that information. These rights are subject to some exemptions
which have to be taken into consideration before deciding what
information it can release.
- New environmental information regulations Jan 2005 enable similar
access to environmental information as under the Freedom of Information

- Under the Data Protection Act 1998, you are also entitled to access
your patient medication records or any other personal information held

Classes of Information
All information at Vantage Pharmacy is held, retained and destroyed in
accordance with NHS guidelines. Our commitment to publish information
excludes any information, which can be legitimately withheld under the
exemptions set out in the NHS Openness code or Freedom of Information
Act 2000. Where individual Classes are subject to exemptions, the main
reasons are, for example, the protection of commercial interests and
personal information under the Data Protection Act 1998. This applies
to all Classes within the publication scheme. The information on this
Scheme is grouped into the following broad categories:

The Leicestershire County & Rutland Primary Care Trust has made
arrangements with Vantage Pharmacy for the provision of pharmaceutical
services under section 41 of the National Health Service Act 1977. The
Pharmacy is contracted to provide a range of NHS services and a full
list of these appears in Class 2.

Measurement and fitting for trusses available on site.
We also provide the following services without NHS remuneration:
· Free prescription delivery service for local housebound
patients.
· Advice from the pharmacist
Dispensing of private and veterinary prescriptions.
Lifestyle advice and diagnostic testing
It is one of the requirements of the practice allowance paid to us by
the primary care trust we contract with that we have a practice leaflet
available detailing all our services. Please ask the pharmacist for a
copy.
Our inclusion in the Pharmaceutical List of the primary care trust is

3. Financial and funding information
(Funding details and charging policies)
Some patients are exempt from paying NHS prescription charges
– please ask for leaflet HC11).For those patients who pay and
have regular repeat prescriptions they may be able to save money with a
NHS pre-payment certificate. These charges are set annually by the
Department of Health

The prescription charge is a tax paid directly to the NHS. It is not
received by the pharmacist and does not form part of any remuneration
to the pharmacy for NHS pharmaceutical services. The average ingredient
cost of a dispensed item is £10.80 and pharmacies receive an
average return of £1.30 per dispensed item. Further
information on the cost of drugs and appliances can be found in the
Drug Tariff, available from The Stationery Office. The Prescription
Pricing Authority (www.ppa.nhs.uk) produces the Drug Tariff monthly.
Some information in this class may not be released because it is
confidential or commercial information or its release may be
prejudicial to the conduct of the pharmacy’s affairs.

5. Regular publications and information for the public
Guidance and information leaflets
There is a range of heath promotion leaflets we display in the pharmacy
as part of our requirements from the primary care trust before we can
receive the basic practice allowance payment. These are published from
numerous of sources, mostly non-publicly funded. We occasionally
receive information from the local primary care trust which we make
available to the public.

Our complaints policy is in line with the NHS Pharmaceutical Services
Regulations 1992 and the NHS Complaints Policy. We would hope any
complaints are directed to us in the first instance so we can resolve

7. This publication scheme
In this class we will publish any changes we make to this publication
scheme, the criteria on which our information management policies are
made and a referral point for all enquires regarding information
management generally in the practice. We will also publish any proposed
changes or additions to publications already available.
Cost of Information
For the most part, we will charge you only for hard copies or copying
onto media (e.g. CD-ROM). Some information is available free, but for
others there may be a charge. The charges will vary according to how
information is made available. Charges are as follows:
a) Via our website – free of charge, although any charges for
Internet Service provider and personal printing costs would have to be
met by the individual.
For those without Internet access, a single print-out as on the website
would be available by post from :

or by personal application from vantage pharmacy.
However, requests for multiple printouts, or for archived copies of
documents, which are no longer accessible or available on the web, may
attract a charge for the retrieval, photocopy, postage etc. We will let
you know the cost and charges that will have to be paid in advance. We
will not provide printouts of other organisation’s websites.
b) Leaflets and brochures – free of charge for leaflets or
booklets on, for example, services we offer to the public. A list is
available in Class 4.
c) “Glossy” or other bound paper copies, or in some
cases a CD-ROM, video or other mediums are for charge as in our
publication lists available from Colin Pickles
